Shared decision making and chronic diseases

چکیده
Chronic diseases are the major cause of death and disability worldwide. Healthcare systems are reforming themselves to improve the health of people with, or at risk of developing, chronic diseases. One of the major goals of these health reforms is to provide both selfmanagement support and decision support to patients so that they become involved and informed [1]. “Shared decision making” (SDM) is described as a process in which the health professional and patient go through all phases of the decision-making process together and in which they share the preference for treatment and reach an agreement on treatment choice [2]. It has been positioned between a paternalistic model, where the health professional assumes the leading role in treatment decision-making, and an informed patient choice model where the health professional’s role is limited to providing information and the patient is responsible for treatment decision-making [3]. SDM is recommended because of its potential to improve the quality of the decision-making process of patients for treatment choices that are informed and value-based, adherence to treatment decisions and ultimately, patient outcomes. It is one of the elements commonly considered important in patient-centered care, a gold standard to improve the quality of chronic disease care. Recently, SDM has also been advocated as a promising strategy to promote effective knowledge translation (KT) between patients and their health care provider. In that context, a SDM approach to KT is defined as a process that is embedded in a specific relationship and by which both the health care provider and the patient influence each other’s cognitions, emotions and behaviours, and come to agreement about a decision [4]. Although there is a growing clinical interest in SDM, it is still not widely adopted by health professionals. Frequently reported barriers include time constraints, lack of applicability due to patient characteristics and lack of applicability related to the clinical situation. Otherwise, factors such as the health professional’s motivation, positive impact on the clinical process, and positive impact on patient outcomes are often reported as facilitating the adoption of SDM [5]. Theory-based approaches are suggested to better inform the design of SDM implementation interventions, as they provide insights on factors that may influence the adoption of SDM by health professionals and patients in a given context. In conclusion, SDM is increasingly regarded as an ideal chronic disease care strategy. SDM may enhance adherence to treatment decision, which is a major public health issue in chronic care.

The Association of Patient Chronic Disease Burden and Self-Management Requirements With Shared Decision Making in Primary Care Visits
BACKGROUND Shared decision making (SDM) is associated with positive health outcomes and may be particularly relevant for patients with chronic disease. OBJECTIVES To investigate whether (1) patients with chronic diseases, particularly those requiring self-management, are more likely to engage in SDM behaviors than patients without chronic diseases and (2) patients with chronic diseases are mo...
متن کاملChoosing wisely and beyond: shared decision making and chronic kidney disease.
In 2012, the Kaiser Permanente Area Medical Directors of Quality decided to sponsor analytic activities to improve shared decision making for patients with chronic kidney disease. The objective was to move shared decision making for renal replacement therapy or maximal conservative management upstream rather than waiting until the patient presented to the emergency room requiring acute dialysis...

Background: The CKD population is becoming increasingly elderly with multiple comorbidities. For this reason, accurate predictive information related to the progression into ESRD, mortality, and functional decline is critical to allow for optimal shared decision making (SDM). Summary: This review will assess the current literature on the methodologies for the estimation of prognosis and prognos...
متن کاملImplementing shared decision-making in nutrition clinical practice: A theory-based approach and feasibility study
BACKGROUND There are a growing number of dietary treatment options to choose from for the management of many chronic diseases. Shared decision making represents a promising approach to improve the quality of the decision making process needed for dietary choices that are informed by the best evidence and value-based. However, there are no studies reporting on theory-based approaches that foster...
متن کاملShared decision-making based on different features of risk in the context of diabetes mellitus and rheumatoid arthritis
There is an increased awareness about patients' involvement in the clinical decision process where uncertainty is an unavoidable condition. The impact of psychological factors like risk aversion, risk aversion and time, asymmetry in risk aversion, and risk and control on shared decision-making is discussed. In addition to differences in risk estimates, doctors and patients may exhibit a differe...
متن کاملPatients' preferences regarding shared decision-making in the treatment of inflammatory bowel disease: results from a patient-empowerment study.
BACKGROUND Shared decision-making is gaining favor in clinical practice, although the extent to which patients want to be involved in choosing their treatment varies substantially. Because data are lacking on the preferences of patients with chronic diseases such as inflammatory bowel disease (IBD), we wanted to assess IBD patients' preferences about being involved in such decisions. METHODS ...
